            Our admissions coordinator will be delighted to review our admission packet with you and assist you with any questions or concerns that you may have. She will also be able to assist with financial matters such as Medical Assistance, Applications, Social Security, Insurances, and Pensions.

            Our primary goal is to see that you are well informed and satisfied with the information that has been given to you.

            Resident roommate compatibility is constantly being strived for so that the optimal level of satisfaction can be achieved. This satisfaction helps you and your family to better adjust to your new environment - your new home.

            To further assist with this adjustment period as well as other psychosocial needs, we have qualified and well experienced Social Workers on staff. Although they are part of Golden Hill staff their primary role is to serve as resident advocates. Social Services are available to all residents and their families. Reassurance and emotional support is often needed when making a major transition in your life. We recognized that making a decision for nursing home placement is indeed a difficult period of transition.

            Psychosocial needs will be identified through visits with both residents and families as well as through interdisciplinary efforts of the staff at Golden Hill working together as a team.

            Whether your stay is respite, short term or long term, the social worker will be able to assist you with all discharge plans. They are responsible for making any necessary referrals such as Area Agency on Aging, meal services, transportation services, private duty services, nursing services and alternate placements if necessary or desired.

            A resident is continuously evaluated regarding capabilities including mental and physical functions. This process of evaluation helps to ensure good quality of care, appropriateness of placement and overall acceptance of the facility.

            The Social Service Director maintains a rapport with community agencies and is able to serve as a referral system for the resident as needs arise.

            Once again our primary goal is to assist you and your family with the entire admission process with the hope of making an often difficult situation into a pleasant experience.
