Culture Change Model

Golden Hill professionals have initiated a model of nursing home care known as “culture change.” The culture change model encourages residents to make independent decisions that promote familiar lifestyles within the community.

The Golden Hill community is comprised of four distinct “neighborhoods.” Each neighborhood is made up of large rooms, common lounge areas, a unique dining area and a caring, competent staff. In their neighborhoods, both residents and staff are empowered to make choices and decisions that directly affect them. The goal is to enhance the human mind, body and spirit through companionship and compassionate care in a homelike atmosphere.

A cooperative team approach to care allows residents and their families to make personal decisions to maintain comfort and dignity for all involved.

This approach offers a refined homelike environment and provides for the continual care of loved ones. This is the reassurance that Golden Hill offers.

The staff strives to ensure that one's "quality of life" is maintained in everything done. We offer ongoing support to our residents, their families and the community regarding issues such as advance directives, residents’ rights and education on various health care topics. Through Care Plan Meetings and everyday interaction, residents, families and friends are directly involved in the planning of the care. Rehabilitation activities and nurse therapy services strive to maintain the highest level of functioning possible for residents.
